Athletics & Fall Sports for Grades 4-8
Overview
If your student is interested in taking part in a sport for SY2024-2025 please read the important information below that includes the sports program at Oyster-Adams and the submission of required forms in order to be cleared for athletic participation.
Fall Sports
The following sports will be offered at the start of this school year:
- Elementary school boys’ cross-country (4th & 5th grade)
- Elementary school girls’ cross-country (4th & 5th grade)
- Middle school boys’ cross-country (6th-8th grade)
- Middle school girls’ cross-country (6th-8th grade)
- Middle school boys’ soccer (6th-8th grade)
- Middle school girls’ soccer (6th-8th grade)
- Middle school girls’ volleyball (6th-8th grade)
*Oyster-Adams does not have a baseball team, but middle school students are allowed to play baseball at other DCIAA schools. We have had kids play for Hardy MS and MacFarland MS.
**Game or practice days for Fall soccer or volleyball are still pending.
Tryouts
If you are new to the community and would like your student to try out for the following sports, please contact our athletic director, Stuart Logan at [email protected]
- MS soccer (girls)
- MS soccer (boys)
- MS volleyball (girls)
**Cross-country is also a Fall sport but there are no tryouts.
Required Health Forms to Participate in Oyster-Adams Sports
Every school year parents must submit athletic papers for a student to participate in a school sport. You do this at the DCIAA Portal.
Please submit your SY2024-25 forms now.
If your student played on a team this school year (SY2023-24) you must submit new forms for them to participate next year (SY2024-25). If your student did not play on a school team in SY2023-24 but she plans to try out / play on a school team in SY2024-25, you must submit new papers.
2024 Fall sports begin the first week of school. Your athlete will not be placed on a roster until her forms are submitted and cleared.
Three forms comprise the Participation Packet:
- Consent for Athletic Participation
- Student-Athlete Data & Emergency Treatment Information
- Concussion & Sudden Cardiac Arrest Information.
You also will be prompted to upload a current Universal Health Certificate (UHC) representing a physical that was conducted within the past calendar year.
If your student’s UHC is valid and in the DCIAA system because he played a sport in SY2023-24, you do not need to upload that same UHC. BUT YOU DO NEED TO SUBMIT A 2024-25 PARTICIPATION PACKET. Later, when that student gets a new physical and corresponding UHC, please go to www.thedciaa.com and re-submit the 2024-25 Participation Packet (it will autofill) along with the updated UHC.
To submit forms, go to www.thedciaa.com. Select “Athletes”, then “Participation Forms”, then “MS/ES Participation Packet”. Fill out the Participation Packet online, then submit it along with a pdf of the current UHC. When doing so, please keep scrolling down to select Oyster-Adams when prompted for Athletic Director and Athletic Trainer.
Important New Information
DCIAA informed the Trainers in March 2024 that (as was always the case) parents must fully complete these forms and leave nothing unanswered. Parents must complete and sign Part 1 of the UHC AND they must completely fill out the AT-13, AT-14, & Concussion forms. DCIAA has already rejected several submissions since March. There can be no blanks other than those allowed by checking a related “N/A” box.
Often omitted but required information includes the following:
Emergency Form (AT-14)
- Two emergency contacts
- Complete health insurance information, including policy number, insurer phone number, policy effective date...
Universal Health Certificate (UHC)
- Complete Part 1 (every blank) and sign/date, including health insurance information
- Doctor office must complete Parts 2-4, including date of health exam in Part 2, check yes box in Part 4 for “This child is cleared for competitive sports”, doctor office stamp in Part 4, and doctor or nurse practitioner must sign and date.
A physician, nurse practitioner, or doctor of osteopathy must perform the physical and sign the required forms. DCIAA does not accept physicals conducted by others, e.g., physician’s assistants and nurses. Moreover, nurses or physician’s assistants cannot sign the UHC for the doctor, nurse practitioner, or doctor of osteopathy.
CVS Minute Clinic and other walk-in clinics perform sports physicals. If you go that route, please print out a blank UHC from the Portal, fill out Part 1 and sign, and take it with you for the doctor to complete.
