- OSEE School-Based COVID-19 Testing Consent Form
Update: Students will now be automatically enrolled in the testing program which supports asymptomatic testing, symptomatic testing and testing for close contacts. Please disregard the COVID-19 testing consent forms previously shared in messages from DCPS. You can read more here for more information.DCPS will be doing saliva-based PCR testing for a random sample of 10-20% of students who have consent forms on file to be part of the testing pool. In order to catch and squash any possible COVID cases at school, we really need people to consider opting in and signing the consent form. Please fill out this consent form (or here in Spanish) and email it to Evelyn (Grades PK4-3rd) or Susana (4th-8th). - Ask, Ask, Look (ALL)
This is the form that has to be completed at home to screen students and has to be presented at the entrance each day. Those who did in-person instruction last Spring for Term 4 will remember filling it up every morning.
