DCIAA (DCPS Athletics) will offer free sports physicals on Saturday, September 24, 9:00-1:00, at Eastern High School [1700 E. Capitol St., NE]. This is for DCPS students who will be in 4th-12th grade in 2022-23. You do not need to make an appointment.
If you go, you must first print out a blank 2022-23 Universal Health Certificate at https://www.thedciaa.com/participation-forms [https://www.thedciaa.com/_files/ugd/98fcc7_a56d182b64b14035a0ec624f373f2ee1.pdf] Fill out Part 1, sign, and take the UHC w/ you to Eastern HS. The same applies if you are getting the physical at your child’s regular doctor or at a clinic (e.g., Glover Park Minute Clinic). You need to fill out Part 1 first and take the form with you for the doctor or nurse practitioner to complete and sign.
